4 RA cases today- All different

In general, on average I treat one patient each day using Relative Analgesia (RA) or "Happy Air".  
Also known as Inhalation Sedation (I.S.

Today I found a use for it for four of my patients, all for different reasons.

Patient 1: A man in his fifties with very poor dental health. He is about to lose an upper central incisor in an otherwise intact upper arch of teeth. He fears the noise of the drill. Amongst other preventive and hygiene treatment, he needs 4 teeth removing and two, six-unit anterior bridges made UL3-UR3 and LL3 -LR3. All the lower incisors are already missing.
 
Today's visit was to carry out his new patient clinical examination (having already had a meet and greet non-clinical visit to discuss his issues.) AND to give him a test drive of the "Happy Air" for us both to assess his reaction.

He took to it rather well but he is clear in his own mind that he needs the amnesia that IV Midazolam can offer and the reduced awareness of what is happening and of time passing quickly as he needs several long visits.

His treatment may take the best part of 12 months and will no doubt entail at least 2 x IV sessions and probably several sessions with happy air for the simpler stages of rehabilitating his dental condition.
Still, using a toothbrush, a little more systematically, will be a good start!!

Patient 2: This late 20s young man appeared at short notice for the first time today with severe toothache in LR6.


 He hadn't seen a  dentist for at least 10 years through fear. Fear of being told off and fear of unpleasant smells. We discussed the idea of I.S. He liked it,. He gave informed consent and we were able to sedate him and give local anaesthesia in about 5 minutes.

We proceeded to undertake a vital pulpotomy (removed the top part of the nerve) and placed a temporary filling.

He recovered within 3 minutes and said that was the best dental visit he had ever had. He'll be coming back for a full examination as a new patient. He will need at least 2 root fillings, a 3-unit bridge and at least one crown.  I am confident that he'll be back.

Patient 3: This was a 14-year old referred to me by another practitioner as she would not allow him to give her a local anaesthetic injection for a specific tooth needing a filling. She came to see me a week ago for a discussion and assessment and to have a test run of the happy air without needing to open her mouth. She got on very well with this approach and returned with her mother today. 
Again she sedated very nicely and I used the single tooth anaesthesia method using
The Wand Plus®. The tooth was badly decayed but this is not the place to discuss that detail.

Both she and her mother were delighted with the outcome, which is just as well as she is likely to need a root-filling at some time in the not so distant future. She will be quite happy to return. She';ll need to for the cavity in LR7 too!!

Patient 4:

Finally we saw a 20 year old for her 3rd visit. Two fillings in upper molars, both deeper than would have been obvious. Under the influence of "RA".  Totally relaxed, it was a pleasure to carry out her treatment.

RA Course Report 24th March 2012

Relaxing on a beautiful spring day at home in Newbury following our 29th RA course, it is time to reflect and check the feedback.

This time the one-day course was held in our new education suite and so everything was different for us and in truth, only just ready in time with plumbing still going on the day before.
 

We decided to use both rooms with one set up for the interactive, didactic teaching 















and the other with a massage bed , doubling as a dental chair, for the practical demonstrations, using a mobile stand for the equipment.



I must say that on reflection, although this arrangement did work and allowed us to demonstrate the technique very nicely, talking it over with Chris, we feel that using my treatment room with a bona fide dental chair and our neat, fixed installation, is probably more akin to what delegates would be used to.  In that respect, as well as others, it differentiates our offering from that which SAAD are able to provide.

So for future courses, we will revert to using my treatment room for the practical demonstrations, including the one with a patient, which will still be patched through to the new seminar room via a-v links.

For me, one of the highlights of these demonstrations is when I offer the patient the opportunity to speak unscripted to the delegates . They only have a few seconds to say yes or no and invariably ( so far) accept the invitation.

This allows the patient to say how they felt about the experience immediately afterwards and also for the delegates to fully appreciate both the effectiveness of technique and just how rapidly the patient recovers and how lucid they are within, a few minutes of " switching off" the nitrous
oxide.

Feedback Results


We had 9 delegates. 6 dentists and 3 dental nurses.
3 dentists from different practices in Ireland and 2 from Leeds with 2 of their dental nurses.
None of these have used sedation or have seen patients treated under conscious sedation.
Finally, 1 GDP and his nurse from Hereford, who is experienced in IV sedation and had tried but gave up on RA some years ago.

Of the 9, 5 chose to give additional comments: Here they are, warts and all, in no particular order.
 
1) "The course was very informative, interesting and fun. Looking forward to practicing it"
 
2) "Thank you very much for an extremely informative day"
 
3) Please do not use yellow or light colour on the slides which are to be printed as it doesn't show up"
( My sincere apologies to any delegate whose handout was unclear, we did have some printing issues beforehand and I will of course be sending better copies to any delegate who requires it)
 
4) "Really enjoyable course. Enjoyed both the theoretical part and the hands-on element. Dr. Charon makes RA seem very straightforward. Chris and the other nurses were very kind also.
 
5) I was very impressed with what I felt was a unique course, small group size and 'special interest' experience from Richard, Chris and the team. I would and shall gladly recommend to colleagues interested in R.A.

Feedback scores:

Venue Excellent, 2: v .good 7 (Need to improve central heating. Too cool at the start and a tad warm by the end of the day)
Relevance: Excellent 8, v. Good 1
Audio-Visual: Excellent 5, v. Good 4 ( a fair reflection. We had a couple of minor glitches)
Speaker: Excellent:8 v. Good 1

So room for improvement on the audiovisuals. A few of the 490 slides to be edited. I'll be seeking further clarity on this from the delegates.

Summative assessment results

 All delegates completed a short MCQ by way of a summative assessment of the day's teaching.

7 delegates scored 83%
1 delegate scored 91%
1 delegate scored 100%

So all in all a pretty successful day's learning and vCPD and very helpful feedback too.

Education Facility is Almost Ready

Our new self-contained education facility, adjacent to Richard Charon at Newbury Smilemakers Dental practice, in Newbury, is close to completion. There are 2 rooms. The seminar room can hold 14-16 people with a projector & screen. The other is an ante-room, which could double for a smaller breakout or parallel group of 6-8 people.

Venue for 21st Jan 2012. Update News

6 working days into our education suite refurbishments and it is now clear that the new venue will not be completed in time for Jan 21st. This being the case the venue will be Precision Dental Studio, Rivermead, off Piper's Lane, Thatcham, Berks. RG19 4EP, where will have use of both their Gold and Platinum rooms and their surgery too.



This is a dedicated education suite too, which we have used three times before very successfully.

An Interesting day. RA (Inhalation Sedation) course- Apollonia House Nov 26th 20-11

Now we are safe and sound back home in Newbury it is time to reflect and take stock. The word that keeps coming to  mind is... interesting, in many of the connotations of that word.

Interesting point one: The amazing location of Apollonia house, perched on an outcrop overlooking a valley in the area of Saddleworth Moor with wonderful views in all directions.









Before going further I must now pay tribute to Andy Lane, not just for hosting us at his stunning practice but also for spending almost the entire day in the background, working pretty much non-stop to try to fix several IT problems that descended on us simultaneously like a perfect storm.  Thanks to his efforts most of the main issues were solved though one or two did remain to take the shine off some aspects of the program, it is fair to say.  Still lessons have been learned.  It is always more of a challenge delivering the course away from our home base.

In addition more thanks go out to several people.

First and foremost to my other half Chris, who soldiered-on gamely, to help the day run as smoothly as possible, having gone down with a cold the night before and having been awoken by the hotel alarm going off 3am ahead of our planned 6.30am start.  This early rise ensured we missed the hotel breakfast, to start our set up.

Secondly, a huge thanks to our sponsors, Janet Pickles of RA Medical Services, whose attendance, input and help on the day by bringing duplicate mobile equipment and tubing, allowed us to adapt the equipment to the layout at Apollonia House. Also thanks for her generous sponsorship contribution toward the delegates lunches.  Thanks also go to Jane Walters from Orascoptics, who also attended from 8.00 am and who provided an additional sponsorship contribution.

Thirdly, more huge thanks to Eva, Andy Boon's nurse who came in for the day, leaving an 8 week-old baby with dad, to ensure the clinical side ran very smoothly, as well as participating in the learning for the day. Eva was a great help to us and a great ambassador for the practice.

Last but not least, thanks go to Dr. Andy Boon, an associate at Apollonia House, who gamely took on the live treatment on one of his very anxious dental patients, (who had said,"I don't do dentists") whilst being watched by 14 other dentists and two nurses.

I undertook and oversaw the sedation, while Andy and Eva carried out a composite restoration. Despite hiring a camcorder especially for the occasion (as mine had been playing up) Andy Lane and I were unable establish a wired connection, to the projector, in the way I normally do, to relay images of live patient treatment to the delegates in another room.

As the surgeries at Apollonia House have glass walls and we used a corner surgery, with the patients permission, all the delegates could watch proceedings directly with some relayed commentary from me where appropriate.

So, interesting point number two. This was the 26th one-day course I have run and the 5th away from Newbury. The delegates were amongst the most diverse we have had. Two were highly experienced sedationists though mainly with IV sedation. Roy Bennett a colleague I had met before via SAAD and the DSTG, teaches at Liverpool Dental Hospital and has his Dip Sed. so as GDP myself, that was always going to be a challenge!  Roy was very gracious I must say.  The other GDP with IV sedation experience came with her nurse from N. Wales and they were the first Welsh delegates we had had
since I started in 2003. Interesting!

Another GDP delegate, Mick Speakman from Barnsley, was attending for the third time with three new associates. Mick has many years experience himself with RA and so had one of his three associates but they were happy to have a refresher.  We were also joined by a dentist and his nurse who flew up from London and another GDP and his nurse who were driven up from London, that morning, by their practice manager, who delivered them, went off for the day himself and then returned to drive them back to London. A 17 hour day for them!  We also had two dentists working in Aberdeen, one Portugese, one Spanish who had flown in the previous day to learn with us that day.

So it was that Chris and I first met Andy outside the practice at 7.30, still in the dark to unload the equipment and deploy it around the practice.
Back to my opening thoughts. Interesting. The practice itself is interestingly designed.
Glass walls around three sides, a continuous corridor running around all the surgeries leading to and from the reception area.

 Each surgery having a glass wall too.
 Privacy is ensured by a built in blind system, but when the venetian slats are rotated, a view is afforded of the surrounding hills and valleys for mies around.

We did have another interesting challenge though as none of the surgeries therefore have external windows and the external glass walls of the practice have no openings either. Since we require waste gases to be actively scavenged and exhausted out of  the building, we had to come up with a solution for the day's demonstrations.



We did know the situation in advance and Janet Pickles of RA Medical services came armed with about 7 metres of exhaust tubing which Andy Boon succeeded in attaching to the air con vent 5 metres up, by fully extending himself on top of a ladder with me steadying it whilst the delegates, unaware, were having a break. You can see the black tubing snaking its way across the back of the surgery. rather spoiling the aesthetic! The RA machine can be seen in the back right corner.


The next highlights, were also "interesting".  In the afternoon we had our usual live demonstration patient. She had been selected too weeks earlier and had agreed in principal.  I had spoken to her the previous week, to reassure her and fully explain the procedure, to obtain her informed consent. She arrived with a friend for moral support, who by her comments, sounded even more anxious, about dental treatment herself.  However with Chris's excellent support and reassurance she did hesitatingly come through to the surgery. We proceeded to use inhalation sedation and by this time the delegates were observing via half-closed venetian blinds. The sedation proceeded very nicely and then Andy Boon took over; provided the L.A. and proceeded to complete the planned lower premolar restoration, all with a totally relaxed patient.

Interestingly,  as he was completing the last few moments of the procedure, the small oxygen cylinder began to run out (and with that the, nitrous oxide supply too- a safety feature).  I decided that rather than start switching cylinders, since all that was left to do was polish the top of the filling on an anaesthetised tooth, there would be no problem. I reassured the patient that over the next to or three minutes the happy air relaxation would go away but since there was no more injections or drilling to do, all woul be well.  Even so, as Andy finished off polishing the surface of the filling and the RA wore off, eveyone noted how the patient's totally relaxed demeanor changed. Her relaxed arms and hands became defensively folded. She drew up her legs, which had been flat on the chair and relaxed and generally returned to her usual anxious self, for the last minute or so.

Now for the the next interesting event. I asked the patient ( as I usually do) if she would be prepared to tell the delegates about the experience. Very briefly, she said that as a psychiatric nurse, indeed a matron, she liked to be in control but that she had been very surprised at how effective the technique had been in reducing her anxiety and that she would be happy have her treatment that way again.
Not a surprising result to those of us who have seen the technique used successfully but a real eye opener for those new to it. She also said how interesting it had been for her to compare treatment whilst sedated this way with the last 2 minutes when the sedation was wearing off.

We all thanked the patient as she left and then there was more of interest. Andy Boon agreed to come forward to give his story. He has treated this patient several times before,  but it had always been difficult. A defensive tongue, frequent stops and so on. This time, Andy noted a complete change in her.  Her tongue rested out of the way, at the back of her mouth and he was simply able to get on with the job in hand. Perhaps he would be kind enough to say more here soon, if I ask nicely!

At he end of the day's learning some had to leave early to get their transport. I have 9/15 feedback questionnaires returned.  When I have more, I'll publish these too. That will be interesting.

All in all, despite some IT hiccups, I believe everyone had an interesting day!

Inhalation Sedation course in Saddleworth. Ready to Roll!

Everything is set for the course at Andy Lane's practice in Saddleworth on Saturday.
We have a full house. Dentists from Aberdeen to London (two practices), Liverpool, N. Wales (a first), from Barnsley and Mansfield too as well as Andy Boon from Apollonia House. Not forgetting several Dental nurses too.

A Guide to Maintaining Professional Standards in Conscious Sedation for Dentistry 2011

Important documents were issued to delegates at the recent SAAD conference in September now available on-line:  Only "A Guide to Maintaining Professional Standards in Conscious Sedation for Dentistry" need concern dentists, who only provide the newly classified "Level 1" sedation techniques, which include RA, IV with Midazolam, Oral Sedation or Intra-nasal Midazolam.

This document specifies the key areas of training and update required and it stipulates that those already providing Level 1 conscious sedation techniques should update every 5 years.

If a GDP or indeed a competent DCP attends one of my one-day courses, it will provide 7.5 hours vCPD of the 12hours required every 5 years. The other 4.5 hours can be made up at other meetings or journal questionnairres, for example, over 5 years. Not an onerous task.
If there is a demand I can also run a 1/2 day, (4 hours of vCPD)  strictly for those already using RA.

A new venue


I am delighted to announce that having secured the lease to premises immediately adjacent to my practice this month, I am planning to develop a larger seminar suite for my hands-on RA courses,

I expect to be able to accommodate a maximum of 16 people. More news will appear when the premises have been fitted out by the end of January 2012

Inhalation Sedation. Another little success

Vicki is my son's partner.
She came to me as a patient about 2 years ago. She was very fearful though not phobic. Here is her story,

Since then she has had several visits mainly using RA but more recently she has not always needed it.
Today she came for a minor filling which needed local anaesthetic (Using the Wand).

I said "Are you OK without the happy air for this" , she said "yes".
Then, as I prepared for the Wand, she suddenly said , "Actually I know I said I'd be brave but I'm getting panicky- can I gave the happy air after all"

"No problem" said I. Within 2 minutes she was sedated and I continued without so much as a hiccup.
10 minutes later the filling was finished ( it was a quick one)  and 3 minutes later she was leaving the dental chair ready to go back to taking care of my grandson!




There is no other simple sedation technique to overcome such anxiety so quickly in a non-invasive way, with no prior preparation and with such rapid return to normal, allowing patients to return to work, school or childcare. Just another reason that I love RA so much !

A new appointment for the Private VT Project

In addition to my other activities and upcoming articles to do with Inhalation Sedation, I can announce that I have been accepted onto a panel of lecturers, joining some illustrious names in UK dentistry, for the newly created Private Vocational Training project, spearheaded by Dr. Andy Lane, whose Apollonia House dental  practice is hosting my next course in Saddleworth.

My time commitment is not fully settled yet but I believe Andy has in mind that the sedation modules will be delivered to the year 2 Private VTs. 

I would envisage EVERY dentist taking up this route into private practice, would undertake RA training and mentorship through to competency certification. 

An excerpt from "The Competent Graduate" published in 2000,relating to Conscious Sedation in Dentistry states:

" 3) Defining Competence

At graduation, dentists should be aware of the advantages and disadvantages of using conscious sedation techniques and should be capable of providing effective sedation for selected patients undergoing straightforward dental procedures in the primary care setting.
This means that the graduate should:
i) be able to assess a patient's need and suitability for conscious sedation and
ii) possess both the ability and confidence to apply knowledge and skills relating to inhalation sedation (using nitrous oxide and oxygen) and intravenous sedation (using midazolam)."


So this is an exciting new chapter for me.

RA course on 26th November in Saddleworth- Now Full

The course on November 26th at Apollonia House in Saddleworth, near Oldham, is now fully booked.

We have 2 dentists and their Dental Nurses from different practices from London attending. They will be joined by 2 GDPs flying in from Aberdeen, a group of 4 GDPs from Barnsley. This will be the practice owner's 3rd visit to my courses. Also our first GDP and her DN from N.Wales & another GDP from Nottinghamshire plus Andy Boon from Apollonia House and last but certainly not least, my SAAD and DSTG colleague from Liverpool, Roy Bennett. A full-house indeed and a great mix of people. I am very much looking forward to meeting everyone.

Its great to have made a difference

This is call for confirmation of reservations for the next RA course at
Apollonia House, Saddleworth, Nr Oldham on Nov. 26th.

We have a maximum of 15 places and 10 of these are now booked and paid for to date.

Please do let me know if you wish to confirm a place/places.
